Update issue #462: add support for default credentials (username+password in Options dialog, checkbox in Project/Url dialog)

fix debug messages, CTraceToOutputDebugString: a space was missing after __FUNCTION__

After removing an item from the monitored list, force saving the changes.

Project moved to SourceForge, update links and properties.

Optimize for sequential access.

Remove unneeded variable assignment.

Use `empty()` instead of `size()` where possible.

Initialize variables and fix a resource leak.

Found by Coverity.

Store the last-checked revision in a separate variable when fetching the next few log messages so it can be used as the start revision. Also set the last-checked revision to zero so the 'limit' is used.
