Remove unused Flipcause integrations

The shortstat is 22891 files changed, 2789287 deletions(-), and this saves 4876 bytes per file, netting 106 MiB reduction on a checkout. There are 14 files that actually have a button that have not been modified here: 1.5.1/users/index.html and index.html from 1.5.1 to 2.2.5.
